Proforma indemnity
I confirm that I will make all individuals booked under my name or travelling with me aware of these documents, and of their contents. Each person in the group will be required to sign an indemnity form personally at the start of the tour.
I hereby undertake to obey all instructions given by the facilitators or their assigned assistants. I voluntarily assume the risks of taking part on this activity.
I acknowledge that I am responsible for attending to any medical condition which I may have (e.g. illness, allergy, etc.).
No alcohol is allowed during training.
